Fundraising Opt-out/PTO Donation Form




Fundraising is necessary to support programs and events sponsored by the PTO, such as school-wide assemblies, Fun and Field day, and the buses and T-shirts for the class trips. The PTO would like to offer an Opt-Out option which gives you the choice to not participate in fundraising by making a one-time donation directly to the PTO. 

After reviewing this year's PTO budget, we expect to spend approximately $125.00 per student in order to provide these services and events for the upcoming year. If you choose the Opt-Out option, you do not have to participate in any of the fundraising opportunities throughout the year.  You may, however, receive information that is distributed school-wide.  You will also receive information on Family events (i.e., Boogie Bash and Breakfast with Santa), pretzel sales, snack grams and holiday sales which we hope provide fun events for your family to participate.  

The minimum donation to opt-out is $25.00 per student.  If you are interested in the Opt-Out option, or if you do not want to Opt-Out but would like to make a tax-deductible donation, please fill out this form and check the applicable box.  Checks should be made out to “Lindeneau PTO” and can be returned to school.  Please indicate what the money is for with your submission.
